A Clinical Study of Periappendiceal Abscess



Abstract
The collected data of 66 patients who were operated upon under the diagnosis of periappendiceal abscess at the Seoul National University Hospital during the last 6 Years(1976. 7~1982. 6) were studied for some clinical aspects of periappendiceal abscess.
Following results were observed.
1) In 1,245 acute appendicitis patients during the same period, 66 patients(5.6%) were diagnosed as periappendiceal abscess.
2) The male: female ratio was 1:1 and the mean age was 44.6 years.
3) The average symptom duration was 14.1 days as apposed to 4.2 days in simple appendicitis.
4) The average hospital stay was 14.1 days and the hospital stay was not related to symptom duration.
5) Abdominal mass was palpable in 66.2%, over 38¡É body temperature was noticed in 18.2% and leukocytosis over 10,000 was oberved in 66. 7% of patients.
6) E.coli and Streptococcus were the predominant organisms.
7) The diagnostic accuracy of colon study and ultrasonography were 68.8% and 71.4% respectively.
8) Postoperative compications developed in 19 patients(28.8%).
9) Partial colectomy was performed in 10 patients, one due to severe intestinal obstruction, one due to hemorrhagic necrosis of bowel wall, four due to severe adhesion and thickening of cecal wall. But, acceptable rationale was not identified in four patients.
